Q. How do I
enroll my child in CPC?
A child must be 2 years and 9
months by September 1st in order to begin school (three by December
2). Creative Play
Center members have
priority for admission to the regular session during February sign-ups. Alumni families have priority over the
general public. Children will be given
class assignments according to their age group, with exceptions made for
individual developmental needs as determined by the teachers. In general, the morning classes are for the
younger children, and the afternoon classes are for the older children.
Step 1:
Submit an Application for
Membership. You can find the Application
Form on this website under “Documents”.
Complete the Application and return it along with the $15
Application Fee (this is a per child fee and is non-refundable) to CPC
Membership Chairperson, 2323
Pleasant Hill Road, Pleasant Hill,
CA 94523. Make your check payable to Creative Play
Center. You may also obtain an Application for
Membership by calling the Membership Line at (925) 974-6820 and request to have
an Application mailed to you.
Step 2:
The Membership Chairperson will contact you upon receipt
of the Application for Membership and $15 Application Fee to advise whether
your child may be placed in a class or on a waiting list (if the class is
currently full). You will not be
placed on the waiting list or given a class assignment until both the
application and fee are submitted.
If your child can be placed in a
class, you will need to submit a $40 Registration Fee (this is a per
child fee and is non-refundable) to CPC Membership Chairperson, 2323
Pleasant Hill Road, Pleasant Hill, CA 94523. Make your check payable to Creative Play
Center.
Step 3:
Upon receipt of the $40
Registration Fee, the Membership Chairperson will issue you a Username and
Password to log onto the Membership website. You will need a Username and Password to
access the Registration Packet.
Once you login, click on “Documents” then “Registration Packet”.
Download forms 01 to 20. (Fill out the forms and print out 2
copies,
one for your records and one to send in to CPC.) Mail your
completed Registration Packet
and Registration Fees to CPC Membership Chairperson, 2323 Pleasant Hill Road, Pleasant Hill, CA 94523. Make your check payable to Creative Play
Center. You may also obtain these forms by calling
the Membership Line at (925) 974-6820 and request to have a Registration Packet
mailed to you. Step
4:
IMPORTANT--The following information must be submitted before your
child can attend school:
Health, Safety and Emergency Forms—
Written permission must be given before admission that, in
the event of injury requiring medical attention, a child may be attended to at
school, taken to his own physician or to any reputable physician if necessary,
or may be taken by ambulance to a hospital.
A doctor must complete the Physician’s Report. Each child must have been examined within
a year prior to his or her admission day to the school.
TB Tests—must be submitted before you can co-op.
Parents entering Creative Play for the first time must
have an intradermal PPD skin test or chest x-ray. TB tine is not acceptable. For persons 35 years and older, 2 intradermal
PPD TB skin tests are required, at least one week apart. However, if you can provide proof of a
negative PPD within the preceding four years, then only one test is required.
For continuing parents at Creative Play, your intradermal
PPD TB skin test is valid for two years.
If it has expired or will expire during the school year, you will need
to have another test taken. A negative
skin test is also required for your spouse if he/she plans to share co-oping
duties on a regular basis.
Vaccinations—
State Law required all children must be current in their
varicella (chicken pox), diphtheria, pertussis, tetanus, poliomyelitis,
measles, mumps, rubella and Hib meningitis immunizations and must have a
physical examination before entering the school. Immunization is not required when the child’s
physician recommends, in writing, against immunization on medical grounds and
states that the child’s presence will not jeopardize the other children. Exemption may also be obtained if the child’s
parents sign a religious exemption request, and the Department of Health or the
Licensing Agency determines that the child’s presence does not jeopardize the
other children. Other health and safety

Manual.
